Secret Santa Questionnaire Printable

Printable gift-exchange forms in English or Spanish. Two per page to save paper. Hand them out before the draw so nobody gets a third bath set.

Why bother with a Secret Santa questionnaire

The person drawing your name usually knows far less about you than they think they do. Four people in an office could each describe your tastes differently, and the result tends to be something generic and safe. A half-page form removes that guesswork for about two minutes of effort.

The trick is asking about preferences rather than presents. "Things I'd rather not receive" and "favourite snacks" produce better gifts than "what do you want", which either comes back blank or asks for something nobody wants to buy.

How to run it

  1. Print one form per person and hand them out when people sign up.
  2. Collect them before the draw, not after — otherwise the questionnaire is written by whoever drew who, which defeats the point.
  3. At the draw, give each person their recipient's form along with the name.
  4. Hang onto completed forms for next year; half of them will still be accurate.

Setting the spending limit

Put the limit on the form itself, ideally in the heading. An exchange where everyone spends differently leaves somebody feeling either cheap or showy, and it's an awkward thing to raise after the gifts are bought. Entering a limit in the box above stamps it under the heading on every form.

Frequently asked questions

Is there a Spanish version?

Yes — switch Language to Español and every field heading changes, including the instructions.

How many forms per page?

Two, laid out side by side. Each half-page has room for six answers, which fits one form per person without crowding the writing space.

Can I add my own questions?

Not directly on this page yet, but the six defaults cover most exchanges. Write to us via the contact page if the questions don't suit your group.

Is this the same as a wish list?

It's broader. A wish list asks for items; this asks about tastes as well, so whoever draws you can pick something that isn't on any list.
